Biometric Screening: earn 10,000 points. A biometric screening is easy to check on essential health measures such as cholesterol, glucose, and blood pressure. Knowing your numbers is vital to getting you on the path to improving your well-being. If you could not attend an on-campus Wellness/Benefit Fair, you may still complete a biometric screening with your Primary Care Physician (PCP) to receive the well-being credit. Log into the USG Well-being platform with Virgin Pulse > navigate to the Programs page > change your view from Explore to View All from the top menu bar > find LabCorp Biometric Screenings > click #2 Visit your Primary Care Physician. Get the form and instructions here. Click on the download box and print the documents made explicitly for you to take to your PCP. You must print this form, sign it, and take it to your primary care for completion. Instructions on returning the form to LabCorp are located on the same page under Form Submission Instructions. Health Assessment: earn 5,000 points. Employees and spouses may answer health assessment questions online or through your mobile device about your daily nutrition, exercise, sleep and general health habits, emotional state, and coping strategies. At the conclusion of your assessment, you will see your results and be able to measure the improvements to your well-being over time. My Care Checklist: earn 1,000 points for three screenings.
