VACCINE ADMINISTRATIVE LEAVE In an effort to encourage unvaccinated USG employees to schedule their COVID - 19 vaccine appointment by December 31 , 2021 and to thank those who are already vaccinated , the USG will provide employees with 8 hours of administrative leave . In order to make this process as convenient as possible while maintaining service to our students and campus community , employees may request time off for approval through December 31 , 2021 . Employees need to fill out the Vaccine Administrative Leave Form and submit it to their supervisors . You must use Adobe to complete the form electronically . Managers will be required to save the form and enter the time on the employee’s OneUSG timesheet. Our priority is to protect the health and safety of our campus community by encouraging and supporting all of our employees to be vaccinated .
