Supervisor IS YOUR EMPLOYEE LEAVING UNG? Once both the employee and supervisor’s signatures have been obtained on the Departure Clearance Form, the supervisor should e-mail the form to Monica Arrendale. Monica will then send i t out for the rest of the signatures. Supervisors also need to complete a Personnel Action Form or PAF, found here.
